I, the parent/guardian of the above named participant, hereby give my approval for my child/ward to participate, and further assert that I have verified with my child/ward's physician, and in my opinion, my child/ward is physically fit and can participate without limitation in any and all local, regional, national, league/conference, association and team/squad activities, including transportation to and from the activities by a licensed driver.
I am of the opinion that my son/daughter/ward is scholastically fit and would benefit by participation in this program. I agree to submit a copy of my son/daughter/ward's last completed grade, end of year/last completed report card, or a written statement of scholastic fitness from the school administration.
We acknowledge and understand the risks involved in my child/ward playing football, which is a collision sport. The NOCSAE committee has adopted the following warning to be read by, and signed by, both the parent/guardian and participant:
"DO NOT USE THIS HELMET TO BUTT, RAM, OR SPEAR AN OPPOSING PLAYER, THIS IS IN VIOLATION OF FOOTBALL RULES AND CAN RESULT IN SEVERE HEAD, BRAIN OR NECK INJURY, PARALYSIS OR DEATH AND POSSIBLE INJURY TO YOUR OPPONENT THERE IS A RISK THAT THESE INJURIES MAY ALSO OCCUR AS A RESULT OF AN ACCIDENTAL CONTACT WITHOUT INTENT TO BUTT, RAM OR SPEAR, NO HELMET CAN PREVENT ALL SUCH INJURIES."
As the parent/guardian of the above named participant, I agree to assume full responsibility for any and all equipment/uniforms loaned to my child/ward for use in the Ormond Pride football and cheer program. I agree to promptly return, upon request, the uniform and other equipment in as good condition as when received except for normal wear and tear. If I fail to adhere to this policy, I understand and agree that I will be responsible for, and promptly pay the replacement cost of such equipment.
The ideology of youth sports including this program is to promote good understanding and fundamental knowledge of the sport. It is also critical that good sportsmanship, including the ability to always conduct oneself in an appropriate manner of positive accord both on and off the field. It is understood that any incident considered detrimental to the pursuit of this ideology will not be tolerated. It will be addressed in accordance with the statutes of the association, conference, current national affiliation, state and local laws, and may result in dismissal from the program and the inability to participate in any future related activities of the association. This code of conduct applies to all involved with the program including but not limited to, the football players, cheerleaders, sprint participants, parents and guardians.

United Youth Football and Cheer League (UYFL) advises that this form, as with any and all registration/contract forms used by your association, should be reviewed by your local council for compliance with any state and local statutes. This form should be kept on file for a minimum of 7 years, longer in the event of an injury. Please confer with your local attorney for advice as to the appropriate maintenance and storage term for this and all such forms. Due to privacy issues, this form should be stored in a secure location with limited restricted access and/or used for the purpose of medical care only.

We are a non-profit organization run entirely by volunteers to provide a fun, positive experience for the youth of our community. Our goal is to teach the fundamentals of football and cheer/dance as well as teamwork, responsibility, discipline, sportsmanship, and accountability. In order to achieve this goal, every parent and child is expected to abide by rules and guidelines.
I understand that being a Pride Football player/cheerleader is a privilege and I agree to act with self-control and not in a disorderly manner.
I agree to follow the instruction of the coaches and directors without dissent.
I agree to encourage ALL my teammates’ efforts and not criticize mistakes or bully anyone.
I agree to attend all scheduled practices, conditioning drills and games. If unable to attend, I will contact my head coach.
I agree to take good care of the equipment that is checked out to me. I understand that I must return the equipment at the end of the season or upon leaving the program (whichever is first).
I understand that disruptive behavior, foul language, disrespect, defacing property, and failure to follow instructions are grounds for disciplinary action including suspension, dismissal or permanent removal from the program.
I understand that my son/daughter is part of a unique program and that every effort will be made to physically prepare my child for the demand of contact football/cheerleading.
I understand that at no time am I allowed on the practice fields or any game field.
I understand that any conference/meeting with a coach must be prearranged and will take place at the practice facility only, prior to practice and never on a game day.
I understand that open criticism/disrespect of the program and/or coaching staff in public is grounds for disqualification of me and/or my child from the OBPYFC program immediately.
I understand that my child must be picked up from practice and games on time or have prearranged ride share plans.
Parent/Guardians are REQUIRED to fulfill volunteer and fundraising obligations as set by OBPYFC. Each family is also responsible for providing a snack or drink for the team for one game. Parents will have the ability to pay a fee to opt out of the fundraiser/volunteer obligations. Your Team Mom will have the details if you are interested in this option.
Players must be present for 2 practices a week to play in Saturday game. Parents are responsible for bringing and picking up their child from practices. If you need assistance please ask but refrain from making it a habit. All paperwork must be turned into Prior to Regular season practices. Parents/family must work one home game each. chains, concession, gate. Signups at a later date. Each player must me active in Sposorship and Fundraising. We keep it low for YOU!
Failure to return football equipment/uniform will result in $300 charge from the Pride.
